Admissions

We understand that the process by which your loved one is admitted into a skilled nursing facility is multi-faceted and comes with many emotions and stressors. Please don’t hesitate to call, email, or come by our facility with any questions you may have. You're invited to sit down face to face with our admissions personnel, or schedule a tour of our campus. Admissions Process

When considering placement for your loved one in a skilled nursing facility, there are some initial criteria that must be met. These prerequisites are usually determined by your loved one’s physician – whether it’s their regular family doctor in a local clinic or their attending physician in a hospital. All admissions into a skilled nursing facility must be approved and ordered by a physician. We have included a basic, step-by-step admission process below along with a simple explanation of each step for your review. 

 

  • Admissions department receives referral for patient placement. (This referral can come from the patient’s regular doctor, a hospital where the patient is currently undergoing treatment, or a home health or hospice agency.)

 

  • Admissions department obtains facility approval of referral for patient placement on both a clinical and financial level. (Nursing department reviews referral to ensure our facility is the right “fit” for the patient and the business office reviews insurance benefits to ensure coverage of financial obligations are present. This usually takes less than fifteen minutes.)

 

  • Admissions department arranges an admissions meeting with patient or patient’s family. (Upon approval of referral, this meeting is set up to go over the admission paperwork which must be signed prior to admission. This meeting is also coordinated with social services and nursing to begin an initial plan of care and discharge planning upon admission.)

 

  • Resident is admitted into the facility for care. (Once all the physician orders, medication orders, insurance pre-certifications, and other necessary clinical information are obtained from the referral source(s) then the transfer of the resident from home or the hospital is implemented.)

 

The above admissions process is simplified, but we strive to make it as functional as possible for the resident and their family. Our qualified admissions coordinator, social services director, and business office manager will be there to gladly answer questions and help in every way possible. We are here to serve you!
